We also encourage all patients attending our service to complete the 'Friends and Family Test' forms. These very short feedback questionnaires contain a few questions about whether you would recommend the clinic to someone you know. It also gives you an opportunity to make comments about the service you received on the day you attended. These forms are present throughout the clinical areas and at reception - please ask a member of staff if you have difficulty locating a form to complete!
